Working in Partnership
With our patients and residents
The people in our care are our most important partners. We seek to provide them with as much individual responsibility and decision-making ability as possible. We involve them fully in developing and reviewing their individual treatment programmes and, wherever possible, reflect their needs and preferences in those plans.
We invest considerable time in ensuring the smooth transition of patients and residents from their existing placements into our facilities and, similarly, when they are transferred out from our service to other settings, either within our Group or to other agencies. Our goal is to minimise any anxiety they may experience and to support their progress along their care pathway.
With families and carers
We believe the involvement of each service user’s family, relatives or carers, where appropriate and with the consent of the individual, is crucial to the effective care and treatment of that individual. As an organisation we warmly encourage the involvement of this support network.
With referrers
Care Principles is committed to working in partnership with referring clinical teams and agencies to provide services to meet local needs. We actively engage with local commissioners, clinicians, community and social care teams to develop flexible, responsive and tailored services to meet these needs and to provide services as part of an integrated pathway of care. |
