Referrals
Referrals can be made either:
- Online, through our secure referral form
- Printing a pdf copy, completing and posting to our Central Referrals Manager, at Tulip House (Head Office)
- By phone to Tulip House, on 01638 606300; our Central Referrals Manager will discuss the referrer’s specific requirements and pass the referral on to the appropriate unit
In every case, the Central Referrals Manager or Referrals Co-ordinator / Nurse Manager at the nominated unit will be responsible for ensuring a prompt response and for keeping the referrer informed of progress in processing the referral. The Referrals Co-ordinator / Nurse Manager will also be responsible for co-ordinating the admission process when a patient or resident is accepted.
On initial receipt of a referral, we may make enquiries to ensure that the individual meets the general admission criteria and that funding has been secured for the patient or resident. Documentation may be sought for this purpose. Thereafter, a pre-admission assessment visit to the individual by members of the clinical team will normally be made as soon as possible.
The Referrals Co-ordinator / Nurse Manager can also make arrangements for visits to the hospital or specialist care home by prospective referrers and relatives of referred individuals.
